Competency Standard Care Giver

Junior

Care Giver beginners who just graduated from training in Canopy Insan Sejahtera PT. 
Caring for patients who need assistance because the self is old and partially paralyzed.

• Patients who understand, who care giver, whether the duties and responsibilities and what the rights of patients, home health care.
• Understanding the excellent service to patients and families.
• Understanding the psychology of the elderly and the sick.
• Accompany the patient walks, sun drying.
• Be a friend to the elderly / patients.
• Bathing a patient in bed, in the bathroom, in a wheelchair, brushing teeth, washing the patient’s hair.
• Helping small bowel and wear bedpans and urinals.
• Measuring body temperature, blood pressure, pulse, breathing and able to find abnormalities of the examination results.
• Considering weight and height.
• Prepare and serve food to patients.
• Operating the wheelchair.
• Transferring a patient from bed kekursi wheels of the wheelchair and into bed.
• Installation of oxygen.
• How to clean up around the house patients: wipe the glass. sweeping, mopping, cleaning the bathroom and wc and surrounding areas.
• Method of recording patient data: complaints, test results, medication administration, maintenance actions, abnormalities are found, the results of physician advice etc..
• Give medicines by mouth, ointment.

Medium

Already have more skill because it already has a good experience of caring for patients at home or in a hospital or clinic.

• Competent junior level.
• Provide food through the hose (Sonde).
• Mobilization of patients who experienced paralysis.
• Prevent decubitus sores in patients with a long snooze.
• Provide installation of equipment nebulizer to deliver steam to loosen the tightness of respiratory patients.
• Able to perform patient care using cateter.
• Able to perform maintenance condom catheter.
• Being able to give medicines by mouth, ointments, nasal drops, ear drops as directed.

Senior

Bepengalaman been more concerned and have:

• Have a medium level of competence.
• Skilled decubitus wound care.
• Able to handle patients who nag
• Able to handle difficult patients: total paralysis, experiencing despair because the pain was too long, chatty.
• Deabetes treating patients who require insulin injections.

Home Care Nurse Competency Standards

Junior

Able to perform basic maintenance:

• Bathing patients all positions: in bed, sitting, in the bathroom.
• Caring: false teeth, nails, hair, prevention dikubitus patients (injury) for a long snooze.
• Measuring vital signs: blood pressure, body temperature, respiration, heart rate.
• Measuring fluid balance: inputs, outputs.
• Measure: Height, Weight, Circle stomach.
• Doing action: glycerin spray, wind chimneys, issued a bowel movement manually, put a condom catheters, wound care for dry sterile.
• Giving drugs through: vagina, ears, nose, eyes, anus, skin.
• Adjust the position of the patient: sitting, half-sitting according to the patient’s condition.
• Train patients: walking, getting out of bed, the patient effective coughing, deep breaths.
• Hose feed through.
• Helping the patient to vomit.

Medium

• Able to perform maintenance junior level.
• Perform tests to detect dengue fever.
• Installing cateter in women and men.
• Installing the sonde to include food / medicine.
• Installing infusion.
• Understanding the reasons infusion and able to do the monitoring.
• Huknah can perform high and low.
• Caring decubitus with sizable cuts (grade 11).
• Can calculate the appropriate advisory drip doctor.
• Caring for artificial anus.
• Injected with a variety of ways.
• Preventing complications of prolonged bed rest.
• Taking action shaved for surgery and wound care for hygiene.
• Doing suction / aspiration of fluid in the respiratory tract as well as the others.

Senior

• Able to perform maintenance medium level.
• Provide food through the hose (sonde).
• Caring for patients with tracheotomy.
• Suck mucus through tracheostomi.
• Shortening and lifting drain.
• Care puncture location CVP (Central Venous Catheter).
• Being able to measure CVP.
• Being able to install the ECG.
• Ability to operate equipment 2 health: syiringe pump, infusion pump, ECG monitor
• Being able to assess the patient’s condition nd make suggestions regarding the patient’s condition either to the family or doctor.

ICU Level

• Able to perform senior level nursing skills.
• Being able to read an EKG.
• Capable of operating an ventiltor.

Yunior

Care Giver pemula yang baru lulus training di PT Kanopi Insan Sejahtera.
Merawat pasien-pasien yang mandiri yang perlu pendampingan karena sudah tua maupun lumpuh sebagian.

• Memahami tentang siapa Pasien, siapa care giver, apakah tugas dan tanggung jawabnya serta apa yang menjadi hak pasien, perawatan kesehatan rumah.
• Memahami  pelayanan prima kepada pasien dan keluarga.
• Memahami  psikologi lansia dan orang sakit.
• Menemani pasien jalan-jalan, jemur matahari.
• Menjadi teman bagi lansia/ pasien.
• Memandikan pasien di tempat tidur, di kamar mandi, di kursi roda,  menyikat gigi, mencuci rambut pasien.
• Menolong buang air besar dan kecil memakai pispot dan urinal.
• Mengukur suhu tubuh, tekanan darah, nadi, pernapasan serta mampu menemukan kelainan  dari hasil pemeriksaan tersebut.
• Menimbang berat badan dan tinggi  badan.
• Menyiapkan dan menghidangkan makanan untuk pasien.
• Mengoperasikan kursi roda.
• Memindahkan pasien dari tempat tidur kekursi roda serta dari kursi roda ketempat tidur.
• Pemasangan oksigen.
• Cara membersihkan sekitar rumah pasien : melap kaca. menyapu, mengepel, membersihkan wc dan kamar mandi dan sekitarnya.
• Cara pencatatan data pasien: keluhan, hasil pemeriksaan, pemberian obat-obatan, tindakan perawatan, kelainan yang ditemukan, hasil advis dokter dll.
• Memberikan obat-obatan melalui mulut, salep.

Medium

Sudah memiliki ketrampilan lebih karena sudah memiliki pengalaman merawat baik dirumah pasien maupun di rumah sakit atau klinik.

• Memiliki kompetensi  level yunior.
• Memberikan makanan lewat selang (Sonde).
• Melakukan mobilisasi pasien yang mengalami kelumpuhan.
• Mencegah luka decubitus pada pasien yang lama tiduran.
• Memberikan pemasangan alat nebulizer untuk memberikan uap panas untuk melonggarkan pernapasan pasien yang sesak.
• Mampu melakukan perawatan pasien yang menggunakan cateter.
• Mampu melakukan perawatan kondom kateter.
• Mampu memberikan obat-obatan melalui mulut, salep, tetes hidung, tetes telinga sesuai petunjuk.

Senior

Yang bersangkutan sudah lebih bepengalaman dan memiliki :

• Memiliki kompetensi level medium.
• Terampil melakukan perawatan luka dekubitus.
• Mampu menghandle pasien yang cerewet
• Mampu menghandle pasien-pasien yang sulit: lumpuh total, mengalami keputus-asaan karena sakit yang terlalu lama, cerewet.
• Merawat pasien Deabetes yang memerlukan suntikan insulin.

Standar Kompetensi Perawat Home Care

Yunior

Mampu melakukan perawatan dasar :

• Memandikan pasien semua posisi : di tempat tidur, duduk, di kamar mandi.
• Merawat : gigi palsu, kuku, rambut, pencegahan pasien dikubitus ( luka) karena tiduran lama.
• Mengukur tanda-tanda vital : tekanan darah, suhu tubuh, pernapasan, detak jantung.
• Mengukur keseimbangan cairan : input, output.
• Mengukur :Tinggi Badan, Berat Badan, Lingkaran perut.
• Melakukan tindakan : semprot glycerin, cerobong angin, mengeluarkan buang air besar secara manual, memasang kondom kateter, merawat luka kering  steril.
• Memberikan obat melalui : vagina, telinga, hidung, mata, anus, kulit.
• Mengatur posisi pasien : duduk, setengah duduk sesuai dengan kondisi pasien.
• Melatih pasien : berjalan, turun dari tempat tidur, pasien batuk efektif, napas dalam.
• Memberikan makan melalui Selang.
• Menolong pasien muntah.

Medium

• Mampu melakukan perawatan level yunior.
• Melakukan tes untuk mendeteksi Demam berdarah.
• Memasang cateter pada perempuan dan pria.
• Memasang sonde untuk memasukkan makanan/obat.
• Memasang infus.
• Memahami tentang alasan pemasangan infus serta mampu melakukan pemantauan.
• Dapat melakukan huknah tinggi dan rendah.
• Merawat dekubitus dengan luka yang cukup besar( grade 11).
• Dapat menghitung tetesan infus sesuai advis dokter.
• Merawat anus buatan.
• Menyuntik dengan bermacam-macam cara.
• Mencegah komplikasi tirah baring lama.
• Melakukan tindakan mencukur untuk tindakan operasi maupun untuk Hygiene perawatan luka.
• Melakukan suction/ penyedotan cairan pada saluran pernapasan maupun yang lain.

Senior

• Mampu melakukan perawatan level Medium.
• Memberikan makanan melalu selang (sonde).
• Merawat pasien dengan tracheostomy.
• Menghisap lendir melalui tracheostomi.
• Memperpendek dan mengangkat drain.
• Perawatan lokasi punksi CVP( Catheter Vena Central).
• Mampu mengukur CVP.
• Mampu memasang EKG.
• Mampu mengoperasikan peralatan 2 kesehatan : syiringe pump, infusion pump, monitor ECG
• Mampu menilai kondisi pasien dn memberikan usulan  menyangkut kondisi pasien baik kepada keluarga maupun dokter.

Level  ICU

• Mampu melakukan ketrampilan perawat level senior.
• Mampu membaca EKG.
• Mampu mengopersikan ventiltor.
